If you have an insurance benefit, please present your insurance card when you arrive at our office. We will make a copy of your card for our files and will contact your insurance carrier to determine your orthodontic benefits. We recommend checking with your employer to make sure you are eligible for orthodontic benefits, since some insurance companies have a waiting period before the coverage is effective. We will also submit a prior authorization to your insurance company on your behalf. Insurance benefits are normally paid over the expected treatment time rather than in a lump sum. If your insurance changes, please inform us so we can contact the carrier to determine if any changes have been made to your orthodontic benefit. If you lose your orthodontic insurance benefit, the unpaid balance will become your responsibility. If that occurs we will work with you to make an adjustment in your payment schedule.
